The application to join Upward Bound is available. Click on the Student Application Link above.
- Students should complete and submit this form. Emails will be auto-generated and sent to the applicant’s parent(s) and counselor with links to submit the other required components.
PARENT/HOUSEHOLD INFO Form (to be completed by the parent/guardian)
- Once the student submits their application, a LINK WILL BE SENT to the parent/guardian’s email address so that household income and education information can be securely submitted. This information is required to determine an applicant’s eligibility for UB services.
SCHOOL COUNSELOR REFERRAL (to be completed by the high school counselor)
- Once the student submits their application, a link will be sent to the high school counselor’s email address so that academic history and test scores can be submitted on behalf of the applicant.
